Reimbursement Claim
In case of an emergency, get the insured admitted and inform Care (formerly Religare) Health Insurance on 1800-200-4488 within 72 hours.
For a planned treatment/hospitalization, call Care (formerly Religare) Health Insurance claims desk on 1800-200-4488 at least 24 hours in advance.
You can submit claim form and needed documents like hospital bills, discharge summary etc within 15 days of discharge.
Complete admission formalities at the hospital and get the patient started with treatment.
Settle all hospitalization expenses. Collect the original hospital bills, prescriptions, payment receipts and invoices, discharge summary, doctor reports, pharmacy bills and other documents from hospital at the time of discharge.
Approach the hospital's insurance desk for help with documents required in reimbursement filing - like medical reports, doctor details and prescriptions, bills and invoices, Medico Legal Certificate, discharge summary etc.
TIP: Refer to policy wordings document to ascertain what all is covered in your policy.

Send duly signed claim form with all documents mentioned in it to Care (formerly Religare) Health Insurance after getting treatment/discharge from hospital. Address is: Claims Department, Care (formerly Religare) Health Insurance Vipul Tech Square, Tower C, 3rd Floor, Golf Course Rd., Sec-43, Gurgaon-122009
Care (formerly Religare) Health Insurance will process the application. They will ask you for pending documents if any or send a rejection letter if the needed treatment isn't covered in policy.
Note that the said documents are sent within time. Payment will only be made for items covered under your policy and in the name of the proposer.
Cashless Claim

You will need policy number, date and time of hospitalization, insured's name, location and nature of problem, and your contact details to register a claim.
Approach the hospital's insurance desk and register your cashless hospitalization with Identity Proof and other documents like Insurance Health Card
Hospital will process your application and assist with the treatment.
Insurance company will process the request and ask for pending documents if any. They will then send an authorisation letter to the hospital or send you a rejection letter if the said treatment isn't covered in policy.
Please pay non-medical charges and expenses not covered in policy before the insured is discharged from hospital. Refer Policy document for this list

Claim process
In case of an emergency, get the insured admitted and inform Care (formerly Religare) Health Insurance on 1800-200-4488 within 72 hours. For a planned treatment/hospitalization, call Care (formerly Religare) Health Insurance claims desk on 1800-200-4488 at least 24 hours in advance. You can submit claim form and needed documents like hospital bills, discharge summary etc within 15 days of discharge.
Complete admission formalities at the hospital and get the patient started with treatment. Settle all hospitalization expenses. Collect the original hospital bills, prescriptions, payment receipts and invoices, discharge summary, doctor reports, pharmacy bills and other documents from hospital at the time of discharge. Approach the hospital's insurance desk for help with documents required in reimbursement filing - like medical reports, doctor details and prescriptions, bills and invoices, Medico Legal Certificate, discharge summary etc. TIP: Refer to policy wordings document to ascertain what all is covered in your policy.
Download the claim form from Care (formerly Religare) website. Send duly signed claim form with all documents mentioned in it to Care (formerly Religare) Health Insurance after getting treatment/discharge from hospital. Address is:

Vipul Tech Square, Tower C, 3rd Floor, Golf Course Rd., Sec-43, Gurgaon-122009 Care (formerly Religare) Health Insurance will process the application. They will ask you for pending documents if any or send a rejection letter if the needed treatment isn't covered in policy. Note that the said documents are sent within time. Payment will only be made for items covered under your policy and in the name of the proposer.

For a planned hospitalization/treatment, call Care (formerly Religare) Health Insurance claims desk on 1800-200-4488 at least 48 hours in advance. You will need policy number, date and time of hospitalization, insured's name, location and nature of problem, and your contact details to register a claim.
Approach the hospital's insurance desk and register your cashless hospitalization with Identity Proof and other documents like Insurance Health Card Hospital will process your application and assist with the treatment.
Insurance company will process the request and ask for pending documents if any. They will then send an authorisation letter to the hospital or send you a rejection letter if the said treatment isn't covered in policy. Please pay non-medical charges and expenses not covered in policy before the insured is discharged from hospital.
